What counts as substantive
A wrong definition, unit, formula, date, source attribution, description of a study population, statement about intended use, or omission that materially changes the takeaway is substantive. So is a chart that does not match its table. These issues are investigated against the original source and corrected promptly when confirmed.
Spelling, punctuation, spacing, accessibility markup, and repaired links may be maintained without a correction note when the meaning remains unchanged. The latest-update date is changed when editorial content is materially revised.
How a correction appears
The inaccurate text or visual is replaced. A note describes what changed and why when a reasonable reader could have relied on the earlier version. The note does not expose personal information from a person who identified the issue. If a page cannot be made reliable, it is removed from navigation and the XML sitemap while it is reworked.
